Candidate Application

Annual Dues—$25; Application Fee—$25 (One-time fee) 

Candidates of The American Society of Colon and Rectal Surgeons must be actively enrolled in an accredited or approved residency program in general surgery, surgical oncology, or colon and rectal surgery. A person who is enrolled as a research fellow in an accredited or approved residency program may also qualify as a Candidate Member. This membership category is renewable annually contingent upon the Candidate Member's continued enrollment in a residency program as verified by the program director.

QUALIFICATIONS

To be eligible to become a Candidate Member of the American Society of Colon & Rectal Surgeons, the applicant must meet the following requirements:

  1. Candidate must be actively enrolled in an accredited or approved residency program in general surgery, surgical oncology, or colon and rectal surgery.
  2. Or the applicant may be enrolled as a Research Fellow in one of these accredited or approved residency programs;

FOR CONSIDERATION

The following items must be submitted for the ASCRS to process your Member Application. Once you complete the online application, and submit the dues payment of $25, please email a copy of the following required documentation to the ASCRS Membership Department at membership@fascrs.org. For additional quesitons regarding membership you may call (847) 607-6410.

Submit a copy of the following: Letter of recommendation from the director of your training program stating that you are actively enrolled in an accredited or approved residency program in general surgery, surgical oncology, or colon and rectal surgery. This letter must include both beginning and end dates of residency.

Member Application

Annual Dues—$325; Application Fee—$100 (One-time fee)

Membership in the American Society of Colon and Rectal Surgeons requires that the applicant has graduated from a medical school accredited by the Liaison Committee on Medical Education (LCME), an accredited school of osteopathy, or an accredited foreign medical institution. Members must be fully trained in general surgery and have completed an accredited general surgery residency or training in an accredited foreign medical institution that qualifies the applicant to practice general surgery in his or her country of residence. QUALIFICATIONS

To be eligible to become a Member of the American Society of Colon & Rectal Surgeons, the applicant must meet the following requirements:

  1. Be a graduate from a medical school accredited by the Liaison Committee on Medical Education (LCME), an accredited school of osteopathy or an accredited foreign medical institution;
  2. Be fully trained in general surgery as evidenced by one of the following:
    1. Certification by the American Board of Surgery, or
    2. Satisfactory completion of an accredited general surgery residency as evidenced by a letter of recommendation from the program director of general surgery which must include beginning and end dates of residency; or
    3. Completion of training in an accredited foreign medical institution which qualifies the applicant to practice general surgery in his or her country of residence.
    4. International applicants must provide a letter from their Professor stating they have completed general surgery training and are practicing surgery with an interest in colon and rectal surgery;
  3. Be fully licensed to practice medicine in a state of competent jurisdiction;
  4. Be of high moral, ethical, and professional standing;

The following items must be submitted for the ASCRS to process your Member Application.  Once you complete the online application, and pay the dues payment, please email a copy of the following required documentation to the ASCRS Membership Department at membership@fascrs.org.  If you have additional questions, please feel free to contact the ASCRS Membership Department at (847) 607-6410. Once all documentation and required fees are received by the Society your application and supporting documentation will be reviewed by the Credentials Committee and then submitted to the ASCRS Executive Council for approval.   

  1. Submit a copy of one of the following:
    1. American Board of Surgery Certificate
    2. Copy of your letter from the American Board of Surgery
    3. Letter of recommendation from the program director of general surgery stating your satisfactory completion of the accredited general surgery residency which must include beginning and end dates of residency
    4. Letter from the foreign medical institution's professor stating you have completed general surgery training and are practicing surgery with an interest in colon and rectal surgery
       
  2. Submit a copy of your Curriculum Vitae
  3. Submit a copy of your current medical license

Fellow Application

Annual Dues—$325; Application Fee—$200 (One-time fee)

Fellowship in the American Society of Colon and Rectal Surgeons requires that an applicant meet all criteria for Members as attested by two letters of recommendation from current Fellows of the ASCRS. Applicants must have served a minimum of two (2) years of Membership and be certified, and maintain such certification, by the American Board of Colon and Rectal Surgery. Applicants must have specialized in the practice of colon and rectal surgery for a least two (2) years immediately preceding applying for Fellowship status. For the complete listing of required qualifications, please see Qualifying Information below.

QUALIFICATIONS

To be eligible for Fellowship in the American Society of Colon and Rectal Surgeons, the applicant must meet the following requirements: 

  1. Meet all requirements for Membership in the Society;
  2. Served a minimum of two (2) years as a Member of the Society;
  3. Complete and sign the ASCRS Fellow Application and pay the Initiation Fee of $200; must be current on membership dues;
  4. Specialize in the practice of colon and rectal surgery for at least two (2) years immediately preceding application for Fellowship.  This two-year period may begin when the applicant finishes colon and rectal training.  
  5. Attend at least one (1) Annual Scientific Meeting of the Society within three (3) years immediately preceding the application for Fellowship. 

CONSIDERATION

The following items must be submitted before the ASCRS is able to process your Fellow Application. Please complete the online application and then submit the required documentation by email to membership@fascrs.org. If you have any additional questions, please feel free to contact the ASCRS Membership Department at (847) 607-6410. Once all documentation and required fees are received by the Society your application and supporting documentation will be reviewed by the Credentials Committee and then submitted to the ASCRS Executive Council for approval. Final approval is voted on during the Business Meeting at the Annual Scientific Meeting.  

  1. Copy of your American Board of Colon and Rectal Surgery Certificate or a copy of the letter from the ABCRS.
  2. Letter of explanation if your practice is not limited to colon and rectal surgery.
  3. Two (2) letters of recommendation from ASCRS Fellows.
  4. Copy of your Curriculum Vitae.
  5. Copy of your current medical license.

International Fellow Application

Annual Dues—$325; Application Fee—$200 (One-time Fee)

Membership in the American Society of Colon and Rectal Surgeons as an International Fellow requires that all applicants meet the criteria for Members as attested to by letters of recommendation from two (2) Fellows familar with the applicant. The applicant must devote at least 50% of their clinical activity to colon and rectal surgery for at least two (2) years immediately preceding submitting their application for International Fellowship. To become an International Fellow the applicant must have attended at least one annual meeting within the three (3) years immediately preceding the application for feloowship and they must have served a minimum of two (2) years as a Member of the Society. For the complete listing of required qualifications please go to the Qualifying Information link below.

QUALIFICATIONS


To be eligible for Fellowship in the American Society of Colon and Rectal Surgeons, the applicant must meet the following requirements: 

  1. Meet all requirements for Members in the Society as attested to by letters of recommendation from two (2) Fellows familiar with the applicant.  These letters must attest that at least 50% of the applicant's clinical activity is devoted to the practice of colon and rectal surgery;
  2. Served a minimum of two (2) years as a Member of the Society;
  3. Complete and sign the ASCRS International Fellow Application and pay the Initiation Fee of $200 and be current on membership dues;
  4. Devote at least 50% of the applicant's surgical practice to colon and rectal surgery for at least two (2) years immediately preceding application for Fellowship;
  5. Have attended at least one (1) annual meeting of the Society within the three (3) years immediately preceding the application for Fellowship;
  6. Have paid current membership dues fees prior to submitting application for Fellowship. 

CONSIDERATION

The following items must be submitted before the ASCRS is able to process your Fellow Application.  Please complete the online application and then submit the required documentation by email to membership@fascrs.org. If you have any additional questions, please feel free to contact the ASCRS Membership Department at (847) 607-6410.  Once all documentation and required fees are received by the Society your application and supporting documentation will be reviewed by the Credentials Committee and then submitted to the ASCRS Executive Council for approval. Final approval is voted on during the Business Meeting at the Annual Scientific Meeting.   

  1. Copy of an equivalent certificate of ABCRS from an appropriate national surgical board, fellowship in Royal Colleges of Surgeons, and/or certificates of higher completion of training in surgery.
  2. Two (2) letters of recommendation from ASCRS Fellows.
  3. Copy of your Curriculum Vitae.
  4. Copy of your current medical license.

Allied Health Professional Application

Annual Dues—$50; Application Fee-$50 (One-time Fee)

Applicants wishing to apply as an Allied Health Professional Member of the American Society of Colon and Rectal Surgery must be a licensed registered nurse or physician's assistant with an interest in colon and rectal surgery. Each applicant must be sponsored by an active Member or Fellow of the Society.

QUALIFICATIONS

Applicants must be a licensed Registered Nurse or Physician’s Assistant with an interest in colon and rectal surgery.  Once all documentation and required fees are received by the Society your application and supporting documentation will be reviewed by the Credentials Committee and then submitted to the ASCRS Executive Council for approval.   

CONSIDERATION

To be considered for an Allied Health Professional Membership in the American Society of Colon and Rectal Surgeons the online application must first be submitted along with the $50 dues payment.  Once your application has been submitted, please send the following documentation to the ASCRS Membership Department at membership@fascrs.org.  If you have additional questions, please contact the ASCRS Membership Department by calling (847) 607-6410.  

  1. Submit a Letter of Recommendation from an Active Member or Fellow of the Society.
  2. Copy of your current License.

Affiliated Scientific Investigator Application

Application Fee—$150  (One-time fee)

To qualify as an Affiliated Scientific Investigator Member of the American Society of Colon and Rectal Surgeons the applicant must hold an academic faculty appointment and have published articles related to diseases or conditions of the small bowel, colon, rectum, or anus. Each applicant must be sponsored by an active Member or Fellow of the Society.

QUALIFICATIONS

To qualify for an Affiliated Scientific Investigator Membership in the American Society of Colon and Rectal Surgeons, applicants must meet the following qualifications:

  1. Hold an academic faculty appointment;
  2. Have published articles related to diseases or conditions of the small bowel, colon, rectum or anus.

CONSIDERATION

Once you have completed the online application and paid the dues payment of $150 (includes yearly dues payment), please forward the following documentation to the ASCRS Membership Department at membership@fascrs.org. Once you have completed the application process, your application and supporting documentation will be sent to the Credentials Committe for review. The Credentials Committee then submits all applications to the Executive Council for approval. The ASCRS Membership Department will contact you once your application is approved for membership. If you have additional questions, please call the Membership Department at (847) 607-6410.
